Build the opening paragraph: identity, demographics, occupation, residence, marital data, special habits, and conditional female/neuro items.
Ask like a clinician, not a robot: “What is your name? How old are you? What do you do? Where do you live?” Then convert it into a clean paragraph.

Complaint
Keep it short: patient’s own words, main complaint, and duration. This becomes your C/O line.
Complaint should not be over-medicalized. Write what brought the patient, then summarize it clinically in one line.

Analyze the complaint, then build a chronological story with related symptoms and prior management.
Core analysis: onset, course, duration, associations, aggravating/relieving factors, effect of treatment, and last attack. Pain/excreta get extra fields.

Checklist changes according to the selected system. Use notes to add positives and important negatives.
In OSCE, related symptoms are where you prove you understand the system, not just the chief complaint.
Past History
Diseases, operations, transfusion, drug history, allergies, and chronic risk factors like DM/HTN/TB/IHD.
For any past disease: ask duration, manifestations, investigations, treatment, and complications.
Family + Social History
Consanguinity, similar conditions, chronic diseases, living conditions, and occupational clues.
Final Review
Review the generated history, missing questions, and OSCE summary. Then copy or print.
